Hereditary spastic paraparesis, or Strümpell-Lorrain syndrome, is a rare disease of the nervous system which can lead to severe loss of mobility. The scientific literature on the subject is very limited. This article describes a physiotherapy program that was designed for a 50-year-old patient who underwent 5 months of rehabilitation consisting of physiotherapy sessions and daily self-treatment. The program resulted in an important reduction in pain and improved scores on the outcome assessment scales. Although the patient’s progressive disease was in an advanced stage and had already been established for many years, through the physiotherapy and self-treatment program it was possible to contain the secondary problems and improve the patient’s quality of life. Physiotherapy and exercises that are proposed to patients must be functional and should provide strategies to compensate for lost or difficult skills.
