Adopting an innovative approach to chronic disease management entails a profound modification also of the role of the physiotherapist. In the new role, the physiotherapist not only takes care of the medical issues directly regarding the patient as an individual but is called on also to interact with the patient’s family and social context. In this new model, the physiotherapist acts as case manager. This present article analyzes the reasons for this evolution and its implications.
