Patellar dislocation is a severe injury characterized by the exit of the patella from the patellofemoral groove and it mostly affects young females during sport activities. In this case report, we discuss an adolescent female with primary patellar dislocation who came to our observation after a period of immobilization. After gathering information from the patient about her condition and how it possibly developed, we proposed conservative management to control the pain and swelling, improve knee function and strengthen the injured leg so as to enable a return to sport activities; the underlying scope was to avoid the need for surgery. Further MRI findings will play a particularly important role in terms of the outcome of the rehabilitation.
