Chronic conditions represent the most significant public health problem at global level. In a series of three articles, we will describe the cultural and organizational changes that need to be implemented in order to cope with this problem. In this first article, we focus on the Chronic Care Model, an approach to chronic conditions universally considered as a fundamental reference point for all health policies.
