Home physiotherapy: the new experience of the Home
Care Offering Units (C-Dom)

Home-based physiotherapy is a crucial component of the Home Care Units (UDO C-Dom) as it allows rehabilitation professionals to work directly in the patient’s home. This intervention focuses on enhancing the patient’s ability to perform daily activities in their living environment, taking into account real-life obstacles and tailoring solutions to the patient’s specific functional and pathological needs, as well as their living and social circumstances.
Specifically in the context of healthcare, the assessment and management of physiotherapy necessitate a precise, methodical, and comprehensive approach. This approach extends beyond addressing only functional impairments or limitations and instead considers the patient holistically, based on the bio-psycho-social model.
Ensuring a highly personalized rehabilitation intervention requires several key steps, including preparing for the initial home visit and session, establishing effective initial telephone contact, and conducting a thorough assessment of the home environment and the person’s functional skills. Special emphasis should also be placed on caregivers, focusing on social-health education initiatives targeted towards them and acknowledging the significant burden of care (including potential psychological strain) they face daily.
A comprehensive initial physiotherapeutic assessment of the patient can help identify any significant requirements for a multi-professional intervention. This may include requesting a medical reassessment and potentially involving other healthcare professionals.
Various professional figures collaborate based on the Individual Rehabilitation Project (P.R.I.) and individual rehabilitation programs (p.r.i.) to collectively enhance the patient’s autonomy and quality of life. To facilitate the organization of a UDO C-Dom, it is highly beneficial to schedule regular multidisciplinary and multi-professional team meetings. These meetings can be conducted in person or remotely and serve the purpose of discussing clinical cases of patients. The goal is to encourage an ongoing and informative exchange of ideas and experiences among the healthcare professionals involved, thereby enhancing their collective expertise.
