Nutraceuticals as an adjuvant to physiotherapy intervention
for the treatment of myalgia from prolonged muscle spasm

Muscle spasms, particularly in the paravertebral region, are a significant cause of musculoskeletal pain and can contribute to chronic pain syndromes through sustained contraction, reduced perfusion, and fibrotic changes. In spinal disorders such as nonspecific low back or neck pain, the muscular component is often underestimated despite its etiopathogenic role. Therapeutic exercise and physical modalities are central in rehabilitation, and nutraceuticals offer a valuable complementary approach, especially in patients contraindicated for NSAIDs or polypharmacy. Key nutraceuticals include magnesium bisglycinate, which modulates calcium channels to reduce muscle hypercontractility; β-caryophyllene, a CB2 receptor agonist with analgesic and anti-inflammatory properties without psychoactive effects; and Boswellia serrata extract, rich in boswellic acids (notably AKBA), which inhibits 5-lipoxygenase and modulates inflammation, improving joint pain and stiffness. Combining these agents with physical therapy may enhance symptom control and prevent chronicity, particularly in cases of myofascial pain, muscle-related headaches, or posture-induced spinal disorders.
